Unstable patellas, we can sort conservatively most of the time (so they don't need surgery), as we can with most unstable joints. Doing this as a party trick, or even just resting/sitting in positions that put unneeded stress on the supporting structures of an unstable joint (and place it close to it's end range) can damage, or promote lengthening, of supporting tissue, and reduce stability. You need to get these surgically repaired, there's really no great way of us managing them conservatively. The more it comes out, the more easily it comes out, to the point at which even just rolling in bed can be enough to dislocate it.
